Receiving Services

How do you receive Home Health services? If you are recovering from an illness, wound, surgery, stroke or other disabling event, ask your doctor prescribe home health care. Our knowledgeable staff can assist you in coordinating all your health care needs in the convenience of your home.

Prepare for home health services before you go into the hospital. Tell your doctor, hospital discharge planner, or social worker that you CHOOSE Terrebonne Homecare to handle your home health needs.

Under the direction of your physician, anyone of age is eligible for Terrebonne Homecare's home health services. Our services provided to patients, which include skilled nursing, wound care, certified home health aides, IV therapy, physical/occupational/speech therapies, social services, are reimbursed by a variety of payer sources, which may include:

  • Medicare
  • Medicaid
  • Worker's Compensation
  • Most Private Insurances
  • Private Pay

Criteria:

  • Physician-authorized plan of care
  • Need for skilled, intermittent home care
  • Individual must be homebound *

* A patient is considered homebound if:

  1. Leaving the home requires a considerable and taxing effort;
  2. Absences from home for non-medical reasons are infrequent and are for short periods of time;
  3. Absences from home are for the purpose of receiving medical treatment, regardless of the frequency; and
  4. The client attends adult day care for the purpose of receiving medical care.
